Depression Therapy: Symptoms, Treatment & How Therapy Helps

Depression can feel like sadness, numbness, exhaustion, irritability, loss of motivation, or simply not feeling like yourself. Depression therapy can help you understand what is happening, interrupt the patterns keeping you stuck, and gradually reconnect with yourself and your life.

When people think about depression, they often think about sadness.

Sometimes depression does feel like sadness. You cry more easily. Things hurt. The future feels bleak.

But sometimes depression feels like nothing.

The things that used to reach you don’t anymore.

Music is just music. Food is just food. Sex sounds like effort. Friends you genuinely love feel far away. You know you should be excited about something—and you aren’t.

Sometimes depression feels like exhaustion.

You wake up tired. Small decisions feel strangely complicated. You stare at an email you need to answer and can’t make yourself begin.

And sometimes depression is almost invisible.

You go to work. Take care of your children. Show up for other people. Exercise. Make dinner. Laugh at the appropriate moments.

Nobody looking at you would necessarily know.

But you know.

Life doesn’t feel like it used to.

Depression can affect mood, energy, concentration, sleep, appetite, relationships, sexuality, motivation, physical wellbeing, and your ability to experience pleasure. The Center for Growth’s original depression material similarly recognizes the emotional, physical, and functional impact depression can have.

If some part of you recognizes yourself here, you don’t have to decide whether you’re “depressed enough” to deserve help.

You can simply begin with:

I haven’t felt like myself lately.

That’s enough.

Depression is a mood disorder that affects much more than mood.

Clinical depression can change the way you feel, think, move, sleep, eat, relate to other people, experience pleasure, and function in everyday life.

It can be mild or severe. Some people experience one depressive episode. Others experience depression periodically or live with depressive symptoms for longer periods.

And depression doesn’t look identical from one person to another.

One person may cry every day.

Another may feel emotionally numb.

Another becomes irritable.

Another stops leaving the house.

And another continues performing at a very high level while privately feeling empty and exhausted.

That’s one reason good depression treatment begins with understanding the person experiencing the depression, not simply checking symptoms off a list.

There isn’t a single way depression feels.

You may notice:

Feeling sad, empty, numb, hopeless, or unusually irritable
Losing interest or pleasure in things you normally enjoy
Feeling disconnected from people you care about
Having significantly less energy
Sleeping much more or much less than usual
Changes in appetite or weight
Difficulty concentrating, remembering, or making decisions
Feeling physically slowed down—or unusually restless
Feeling worthless, ashamed, or excessively guilty
Becoming increasingly isolated
Struggling with work, school, parenting, relationships, or everyday responsibilities
Thinking frequently about death or wishing you could disappear

The original Center for Growth material identifies many of these same symptoms, including diminished pleasure, changes in appetite, low energy, guilt or worthlessness, concentration problems, changes in movement, and thoughts about death.

A website can’t diagnose depression.

But you don’t need a diagnosis to recognize:

Something has changed. I don’t feel like myself. And I’m having trouble finding my way back.

That’s worth paying attention to.

Sadness is part of being human.

People feel sad when relationships end, people die, jobs disappear, children struggle, health changes, dreams don’t work out, and life hurts.

Feeling terrible during a terrible time doesn’t automatically mean you have a depressive disorder.

Depression is generally more persistent and pervasive and may affect motivation, pleasure, thinking, physical energy, relationships, and everyday functioning.

But the distinction isn’t always simple.

Grief can be profound. Depression can follow a recognizable loss. And someone can experience grief and clinical depression at the same time.

So we don’t reduce the distinction to:

Sadness has a reason; depression doesn’t.

Human beings are more complicated than that.

Sometimes depression follows something painful.

Sometimes many smaller stresses accumulate.

And sometimes you genuinely don’t know why you feel the way you do.

You don’t have to figure out which category you belong in before asking for help.

Yes.

Some people become remarkably good at functioning while depressed.

You may go to work, parent your children, maintain relationships, exercise, meet deadlines, and take care of everyone around you.

Nobody would necessarily know how difficult things feel internally.

Sometimes competence hides suffering—even from the person who is suffering.

Instead of asking only:

“Am I functioning?”

It can be useful to ask:

“How much of me does it take to keep functioning this way?”

If you’re getting everything done but experiencing little pleasure, connection, energy, or hope, that matters.

Yes. Depression doesn’t always look quiet or sad.

You may have less patience.

Noise bothers you.

Ordinary demands feel enormous.

You become critical, restless, short-tempered, or unusually sensitive.

You snap at someone you love and then feel guilty afterward.

Or perhaps you aren’t exactly angry.

You’re just done.

Done talking. Done trying. Done making decisions. Done caring about things you know you’re supposed to care about.

Depression can narrow your emotional bandwidth until there isn’t much room left.

Irritability can be especially important to notice in children and teenagers, whose depression may show up through anger, withdrawal, school difficulties, behavioral changes, or physical complaints rather than obvious sadness.

Sometimes there is a clear answer.

Sometimes there are ten answers.

Sometimes there isn’t an obvious answer at all.

Depression can develop in the context of:

Grief. Trauma. Chronic stress. Loneliness. Relationship difficulties. Major life transitions. Physical illness. Hormonal changes. Chronic pain. Sleep disruption. Substance use. Medication effects. Family dynamics. Social isolation.

Longstanding ways of relating to yourself and other people may matter too.

Biology matters.

Psychology matters.

Relationships matter.

Your environment matters.

Your history matters.

And their relative importance is different for every person.

Good depression therapy doesn’t force your experience into a predetermined explanation.

Instead, we get curious.

When did this begin?

What was happening then?

What makes it worse?

When does it lift, even slightly?

What have you stopped doing?

Who do you feel most like yourself around?

What are you telling yourself?

What is happening in your relationships?

What is happening in your body?

And one of our favorite questions:

What still reaches you?

Because sometimes that’s where we begin.

Depression therapy helps you understand the factors contributing to your depression and change the emotional, cognitive, behavioral, relational, or environmental patterns that may be keeping you stuck.

There isn’t one kind of depression, so there shouldn’t be one predetermined treatment for every depressed person.

For one person, the most important work may be getting their body moving and their life structured again.

For another, it may be changing a relentlessly self-critical internal voice.

Someone else may need to grieve.

Another person may need to understand why the same painful relationship patterns keep repeating.

Someone may need help reconnecting with pleasure, sexuality, creativity, friendship, or meaning.

And someone else may benefit from psychotherapy alongside medical or psychiatric treatment.

The technique isn’t the starting point.

You are.

One of the cruel things about depression is that its symptoms can begin reinforcing each other.

You don’t have energy, so you cancel dinner.

You feel relieved that you canceled.

Then you’re alone.

You stop exercising because you’re exhausted.

You stop cooking because it feels like too much work.

You stop answering messages because you don’t know what to say.

You fall behind.

You criticize yourself for falling behind.

Now you feel worse.

So you withdraw a little more.

None of these choices is irrational in the moment.

That’s what makes the cycle so powerful.

Depression therapy helps identify places where we can interrupt it.

Not everywhere.

Not all at once.

Just somewhere.

Then somewhere else.

Depression can become a very convincing narrator.

Nothing is going to change.

I’m failing.

Everyone else handles life better than I do.

I’m a burden.

I’ve ruined everything.

What’s the point?

Cognitive Behavioral Therapy (CBT) helps you understand the relationship among thoughts, emotions, and behaviors. The original Center for Growth treatment model similarly uses CBT to help clients recognize these connections.

But CBT isn’t about replacing:

Everything is terrible.

with:

Everything is wonderful!

Sometimes things really are terrible.

The work is more thoughtful than that.

We might ask:

Is this thought accurate?

Is it the whole truth?

Has depression turned “sometimes” into “always”?

Has one painful experience become evidence about my entire future?

Am I talking to myself in a way I would never talk to another person?

The goal isn’t positive thinking.

It’s more accurate and flexible thinking.

Depression often tells you:

I’ll do it when I feel better.

Unfortunately, waiting for motivation can keep depression going.

Sometimes action has to come before motivation.

Behavioral activation helps people gradually re-engage with activities connected to pleasure, accomplishment, relationships, movement, routine, and meaning.

That doesn’t mean handing an overwhelmed person a giant self-improvement checklist.

It might mean:

Take the shower.

Open the blinds.

Walk around the block.

Eat something.

Answer one message.

Sit outside.

Put music on.

See one person.

Do one small thing that once felt like you.

Then notice what happens.

Small changes count.

Especially when you’re depressed.

Depression can trap you in repetitive thinking.

What’s wrong with me?

Why can’t I get over this?

Why did I do that?

What if I never feel better?

Mindfulness and acceptance-based approaches help you develop a different relationship with these thoughts. The Center for Growth’s original treatment model includes mindfulness-oriented approaches aimed at helping people disengage from repetitive negative thinking.

There is an important difference between:

“I’m having the thought that nothing will ever change.”

and

“Nothing will ever change.”

The thought hasn’t disappeared.

But now there’s a little space around it.

Sometimes that space matters enormously.

Not everything should be solved with a coping skill.

Sometimes depression makes more sense when we understand your relationships, history, losses, identity, attachment patterns, and the ways you learned to survive emotionally.

Maybe anger wasn’t safe.

Maybe achievement became how you earned approval.

Maybe needing someone felt dangerous.

Maybe you became the strong one so early that you never learned what to do when you needed care.

Maybe something happening now is touching something much older.

Insight-oriented and psychodynamic therapy can help identify these patterns and understand how earlier experiences continue to influence present relationships and emotional life.

We aren’t interested in blaming your childhood for your adult life.

We’re interested in understanding your history well enough that your history doesn’t have to keep making your decisions for you.

Depression rarely stays neatly contained inside one person.

You may pull away from your partner.

Lose interest in sex.

Stop calling friends.

Become irritable with your children.

Need reassurance and then feel embarrassed that you needed it.

You may want someone close and simultaneously want everyone to leave you alone.

Then you may begin thinking:

I’m no fun anymore.

They’re tired of me.

I’m bringing everyone down.

So you withdraw to protect them.

And become even more alone.

Sometimes part of depression treatment is learning how to remain connected to other people while being allowed to not be okay.

You shouldn’t have to perform wellness in order to deserve company.

Yes.

Mental and physical health aren’t separate systems.

Sleep problems, chronic pain, hormonal changes, medication effects, substance use, nutritional issues, and some medical conditions can contribute to changes in mood, energy, cognition, and motivation.

The original Center for Growth material appropriately recognizes that medical conditions and medication effects can sometimes contribute to depressive symptoms.

When physical factors may be contributing, appropriate medical evaluation can be an important part of care.

We don’t want to assume that every symptom is psychological simply because you came to therapy.

You are one whole person.

Medication can be helpful for some people with depression.

For others, psychotherapy may be sufficient.

And many people benefit from a combination of psychotherapy and medication.

There is no prize for suffering without medication.

And there is no reason to assume medication is automatically the right answer for every period of depression.

Treatment decisions depend on factors such as the severity and duration of symptoms, previous depressive episodes, prior treatment response, medical considerations, safety, and personal preferences.

Medication and psychotherapy aren’t competing philosophies.

They’re tools.

The question is what helps you get better.

Depressive disorders and depressive presentations can take different forms.

These include Major Depressive Disorder, Persistent Depressive Disorder, depression with seasonal patterns, perinatal or postpartum depression, and Premenstrual Dysphoric Disorder (PMDD), among others.

Depressive symptoms can also occur within other mental health conditions.

Accurate assessment matters.

For example, someone who has experienced periods of unusually elevated or irritable mood, substantially decreased need for sleep, unusually high energy, impulsivity, or other symptoms of mania or hypomania may need evaluation for a bipolar-spectrum condition rather than assuming every depressive episode represents unipolar depression.

The diagnosis isn’t the person.

But understanding the pattern can help guide safer and more effective treatment.

Children don’t always say:

I’m depressed.

They may become clingy.

Irritable.

Quiet.

Explosive.

Reluctant to go to school.

They may complain about headaches or stomachaches.

Stop playing the way they used to.

Have trouble sleeping.

Or simply seem different.

The original Center for Growth material similarly notes school refusal, clinginess, physical complaints, and irritability as possible signs of childhood depression.

Children communicate through behavior and play as well as words.

Treatment should meet them developmentally rather than expecting a child to explain their emotional life like an adult.

Teen depression can be particularly easy to misunderstand.

Adolescence already includes enormous emotional, social, physical, and developmental change.

A depressed teenager may look sad.

But they may also look angry, detached, exhausted, unmotivated, unusually sensitive, or completely uninterested in things that used to matter.

Grades may fall.

Friendships may change.

Sleep may become chaotic.

Some teenagers use substances, isolate themselves, engage in self-harm, or begin expressing worthlessness or hopelessness.

The goal isn’t to pathologize every unhappy teenager.

It’s to notice when something has changed enough—in mood, behavior, relationships, functioning, or safety—that additional support may be needed.

Depression is treatable, although recovery looks different for different people.

And getting better doesn’t necessarily mean waking up one morning suddenly happy.

Often the first signs are much quieter.

You answer the text.

You take the shower without negotiating with yourself for forty minutes.

You laugh—and realize you meant it.

Something tastes good.

You want to see somebody.

You finish something and feel satisfied.

You become interested in sex again.

You get angry about something that matters instead of feeling numb.

You put music on.

You make plans.

You notice the weather.

You become curious.

You imagine next month and realize:

I care what happens.

Those moments may look small from the outside.

Clinically, they aren’t small at all.

They can reflect the return of motivation, emotional range, connection, interest, and capacity for pleasure.

The goal isn’t permanent happiness.

No one is happy all the time.

A full emotional life includes grief, disappointment, anger, boredom, fear, joy, excitement, desire, curiosity, and terrible days.

The goal is range.

To feel sadness without becoming only sadness.

To experience pleasure when something pleasurable happens.

To connect when someone reaches for you.

To rest without disappearing.

To care.

To want.

To hope.

To feel like your life belongs to you again.

Depression can sometimes become so painful that death begins to feel like relief.

You may wish you could go to sleep and not wake up.

You may imagine disappearing.

You may believe other people would be better off without you.

Or you may be actively thinking about suicide.

Suicidal thoughts deserve to be taken seriously.

Tell someone.

You don’t have to wait until you’re certain you would act on the thoughts before asking for help.

If you believe you may hurt yourself or cannot keep yourself safe, seek immediate emergency help or call or text the 988 Suicide & Crisis Lifeline at 988.

Having suicidal thoughts doesn’t make you dramatic, selfish, difficult, or beyond help.

It means you’re hurting.

And you need more support around you while you’re hurting.

We don’t want to reduce you to symptoms.

We want to understand what happened to you.

What happened to your energy?

What happened to pleasure?

What are you withdrawing from?

What are you telling yourself?

What hurts?

What feels empty?

What are you carrying?

What have you lost?

What do you miss?

Who do you feel safest with?

When do you feel most like yourself?

And perhaps most importantly:

What still reaches you?

Maybe it’s your child.

Your dog.

Music.

The ocean.

Work.

Sex.

Humor.

Anger.

Art.

A friend.

A walk.

A memory of who you used to be.

Or maybe, right now, it’s simply the small part of you that found this page and kept reading.

That’s enough to begin with.

There isn’t one way into depression.

There isn’t one predetermined way out.

We start with where you are.

And from there, we help you find your way back into your life.
